This privacy policy is to provide information to you, our patient, on how your personal information (which includes your health information) is collected and used within our practice, and the circumstances in which we may share it with third parties.
Your privacy matters to us
We respect your right to privacy as stated in the Privacy and Personal Information Protection Act 1990 and the Health Record and Information Privacy Act 2002. Unless you allow us to, we will not share your information on our website.
Why and when your consent is necessary
When you register as a patient of our practice, you provide consent for doctors and practice staff to access and use your personal information so they can provide you with the best possible healthcare. Only staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this.
Personal information collected on the website
Online visitors are welcome to provide us with enquiries and feedback. The information you provide on our website — such as your name, email address, contact number, etc. — will only be used to respond to these messages. We will never share your information with another party without your consent.
We handle your personal information with the utmost care and with high regard to safety, integrity, and confidentiality. This information may be stored on our computer medical records system and/or in handwritten medical records.

You have the right to request access to, and correction of, your personal information.
Our practice acknowledges patients may request access to their medical records. We require you to put this request in writing, and our practice will respond within a reasonable time.
We take reasonable steps to ensure that your personal information is accurate, complete, up-to-date, and relevant.
This is why our staff may ask you to confirm that your contact details are correct when you attend a consultation. We request that you let us know if any of the information we hold about you is incorrect or out of date.
How can you lodge a privacy-related complaint, and how will the complaint be handled at our practice?
We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing. We will then attempt to resolve it in accordance with our resolution procedure.
